چکیده

Background and Purpose: There is interest in what happens over time to the thrombus after intravenous alteplase. We study effect of alteplase on structure its impact clinical outcome patients with acute stroke. Methods: Intravenous treated stroke intracranial internal carotid artery or middle cerebral occlusion identified baseline computed tomography angiography follow-up vascular imaging (computed first run before endovascular therapy) were enrolled from INTERRSeCT (Identifying New Approaches Optimize Thrombus Characterization for Predicting Early Recanalization Reperfusion With IV Alteplase Other Treatments Using Serial CT Angiography). movement was classified into complete recanalization, migration, fragmentation, no change. migration diagnosed when site moved distally graded according degrees (grade 0–3). fragmentation a new distal addition primary imaging. The association between also evaluated. Results: Among 427 this study, seen 54% median 123 minutes administration imaging, sub-classified as marked (thrombus grade 2–3 + recanalization; 27%) mild moderate 0–1; 27%). In proximal M1/internal occlusion, associated higher rate good (90-day modified Rankin Scale, 0–2) compared (52% versus 27%; adjusted odds ratio, 5.64 [95% CI, 1.72–20.10]). No difference outcomes M1 distal/M2 improved 90-day change (70% 56%; 2.54 1.21–5.51]). Conclusions: common Marked leads outcomes. dynamics should be further evaluated trials reperfusion therapy.

Aspiration Thrombectomy After Intravenous Alteplase Versus Intravenous Alteplase Alone.

BACKGROUND AND PURPOSE Thrombectomy, primarily with stent retrievers with or without adjunctive aspiration, provided clinical benefit across multiple prospective randomized trials. Whether this benefit is exclusive to stent retrievers is unclear. METHODS THERAPY (The Randomized, Concurrent Controlled Trial to Assess the Penumbra System's Safety and Effectiveness in the Treatment of Acute Stro...

Administering Intravenous Alteplase (Tissue Plasminogen Activator [tPA])

Step 1: Eligibility-The eligibility criteria for patients with acute ischemic stroke within 3 hours of symptom onset include: • An adult (> 18 years of age) • Exclusion of intrancranial hemorrhage by an imaging technique sensitive for the presence of hemorrhage • Arrives at the emergency department in time to be treated within 3 hours of symptom onset • The recommended dose of IV tPA is 0.9 mg/...

Prediction of thrombus resolution after intravenous thrombolysis assessed by CT-based thrombus imaging.

The degree of thrombus resolution directly indicates the effectiveness of a thrombolytic drug. We investigated the degree of thrombus resolution and factors associated with thrombus resolution after intravenous (IV) recombinant tissue plasminogen activator (rt-PA) using thin-section noncontrast computed tomography (NCT). Thin-section NCTs were performed before and immediately after IV rt-PA inf...

Unequal access to treatment with intravenous alteplase for women with acute ischemic stroke.

BACKGROUND AND PURPOSE A recent meta-analysis showed that women with acute ischemic stroke are less likely to receive treatment with intravenous alteplase than men. The aim of this study was to assess sex differences in treatment with intravenous alteplase and to explore the reasons for these differences. METHODS We analyzed data from the Promoting Acute Thrombolysis for Ischaemic Stroke (PRA...
